چکیده

A 60 year old white man was admitted from the emergency department after an unwitnessed collapse and generalised weakness and malaise. He had no medical history of note and was taking no drugs. On clinical assessment he had hypertension, which had not previously been documented, with a blood pressure of 187/91 mm Hg. Blood tests showed severe hypokalaemia (2.1 mmol/L (reference range 3.6-5.0), having been normal (4.7) nine months earlier) and metabolic alkalosis (bicarbonate 38 mmol/L, 22-30). Random blood glucose was 6.0 mmol/L (3.5-7.8). On the post take ward round, he was noted to have clubbing of the nails and a tanned appearance. A 60 pack year smoking history was elicited. Further biochemical tests were requested and, because his chest radiographwas abnormal, chest computed tomography was performed (fig 1). Questions

Mineralocorticoid hypertension and hypokalaemia induced by posaconazole

We describe severe hypokalaemia and hypertension due to a mineralocorticoid effect in a patient with myelodysplastic syndrome taking posaconazole as antifungal prophylaxis. Two distinct mechanisms due to posaconazole are identified: inhibition of 11β hydroxylase leading to the accumulation of the mineralocorticoid hormone 11-deoxycorticosterone (DOC) and secondly, inhibition of 11β hydroxystero...

Chronic hypokalaemia in a hypertensive patient

Hypokalaemia in hypertensive patients is a 'red flag' bringing to mind various classic secondary and genetic causes related to both hypokalaemia and hypertension. We encountered a patient who had an unusual cause for his disturbance that, to our knowledge, has not been described in hypertensive patients.

Liquorice: a root cause of secondary hypertension

We describe a patient presenting with hypertension and hypokalaemia who was ultimately diagnosed with liquorice- induced pseudohyperaldosteronism. This rare cause of secondary hypertension illustrates the importance of a methodical approach to the assessment of hypertension.

Hypertension induced by liquorice tea.

A 45-year-old woman presented to her general practitioner with a 4-month history of hot flushes, sweating and headaches. On examination, she was found to be hypertensive, and blood tests revealed mild hypokalaemia. While awaiting the results of further investigation into the cause of her elevated blood pressure, the patient conducted her own research and identified liquorice tea as the potentia...

Arrhythmogenic potential of diuretic induced hypokalaemia in patients with mild hypertension and ischaemic heart disease.

In view of evidence suggesting an association of mild hypokalaemia with cardiac arrhythmia, the arrhythmogenic potentials of potassium losing and potassium sparing diuretic treatments were compared in a controlled prospective crossover study of 10 patients with mild hypertension and ischaemic heart disease. Mean (SEM) plasma potassium was 4.3(0.06) mmol/l and 3.3(0.07) mmol/l after potassium sp...
